Complications associated with brain damage are the leading cause of death in patients with this pathology. The study analyzed the results of treatment of 57 patients with metastatic brain lesions. Treatment was conducted on Leksell Gamma Knife 4C (Elekta, Sweden) in the period from 2009 to 2012. The average lifespan of the treated patients was 7,2 months from the first radiosurgical operation, local control was achieved in 92% metastatic brain lesions, disease-free three-month survival rate was 73%, six-month 24% and twelve-month 7%. Our data confirm that radiosurgery is an effective method for the treatment of patients with metastasis of radioresistant tumors, including melanoma, to the brain. The treatment increases survival rate and allows to achieve high local control with minimal mortality.</p></abstract><trans-abstract xml:lang="ru"><p>Меланома занимает третье место среди первичных опухолей по частоте метастазирования в ЦНС после рака легкого и рака молочной железы [1, 3]. По имеющимся в литературе данным, метастазы в головной мозг выявляются у 10-70% пациентов с диагностированной меланомой 4-й степени. Выживаемость таких пациентов без лечения варьирует от 2,75 до 4 мес, причем большинство пациентов погибают от осложнений, связанных с метастатическим поражением ЦНС [12, 13, 19, 20]. Долгое время стандартным лечением метастазов головного мозга являлись хирургическое удаление наиболее значимых и больших очагов, а также лучевая терапия на область головного мозга (WBRT) [2, 7-11]). Стереотаксическая радиохирургия стала высокоэффективным неинвазивным дополнением к комплексной терапии и в дальнейшем альтернативой хирургическому удалению и радиотерапии [15, 17], так как позволяет осуществить локальный контроль метастазов головного мозга радиорезистентных опухолей, таких как саркома, карцинома почки и меланома [4-6, 14, 16, 18, 21].
