Currently, there are few randomized studies to identify opportunities for the pelvic perfusion for sarcomas and carcinomas of the pelvis.</p></abstract><trans-abstract xml:lang="ru"><p>Методика изолированной тазовой перфузии позволяет создать высокую концентрацию химиопрепаратов в полости малого таза. Используется при местно-распространенных саркомах мягких тканей, рецидивирующем местно-распространенном колоректальном раке или гинекологической онкопатологии, часто после ранее проведенного комбинированного лечения, включающего химио- и лучевую терапию. При тазовой перфузии используются различные схемы химиопрепаратов, однако включение мелфалана, особенно в сочетании с фактором некроза опухоли, обеспечивает уровень объективных ответов, сопоставимый с результатами изолированной перфузии конечностей. Техника тазовой перфузии улучшается со временем: применяются компрессионные средства, накладываемые выше проекции бифуркации подвздошных сосудов для создания максимальной изоляции региона, совершенствуются сосудистые доступы, подбирается наиболее эффективная комбинация химиопрепаратов.
