The goal of this survey is to show the base of evidence for stereotactic photon radiation therapy (SRT) and proton radiation therapy (PRT) in cranial tumors treatment. It’s well known that PRT takes advantage in oncopediatric practice, in the treatment of benign tumors with long life expectancy and possibly skull base tumors. By decreasing integral body dose PRT lowers risk of secondary malignancies and preserves cognitive function. In other cases PRT prevalence over advanced forms of photon radiotherapy is yet to be proved. To clearly see the difference we need to compare intensity modulated proton and photon radiation therapy. Studies continue.</p></abstract><trans-abstract xml:lang="ru"><p>В свете многообразия современных методов лучевой терапии практические врачи часто не могут правильно оценить преимущества и недостатки различных ее вариантов и рекомендовать больному оптимальное лечение. Задачей настоящей публикации является осветить доказательную базу в отношении стереотаксической фотонной лучевой терапии (в дальнейшем СТЛТ) и протонной лучевой терапии (ПЛТ) в лечении опухолей краниальных локализаций. На сегодняшний день установлено, что ПЛТ имеет преимущества в онкопедиатрии; в лечении доброкачественных опухолей с хорошим прогнозом для жизни; возможно, в лечении опухолей основания черепа. За счет снижения интегральной дозы она позволяет сократить вероятность вторичных опухолей и предотвратить снижение когнитивной функции. При большинстве других клинических ситуаций клиническое преимущество ПЛТ перед продвинутыми формами фотонного облучения не доказано. Для адекватной оценки возможностей этих методов требуется сравнительный анализ фотонной и протонной лучевой терапии с модулированной интенсивностью пучка (IMRT и IMPT).
