Patients were divided into 2 groups: first – 21 patients who received treatment with an isolated limb regional perfusion (ILP), melphalan (10 mg/l for lower limbs, and 13 mg/L for upper limb); second (control group) – 36 patients were treated with conventional methods of mutilating surgery. During the ILP original methodology for monitoring of chemotherapy income levels in the systemic circulation was used.<italic>Results. </italic>Patients undergoing ILP with locally advanced soft tissue sarcomas of the limbs can achieve overall 2year survival rate of 90,5% comparing to 75% in the control group. Local toxicity in most cases did not exceed grade I and II under Wieberdink scale. Systemic grade I–II by CTC scale toxicity was observed in only few cases.<italic>Conclusion. </italic>ILP is a promising method for the treatment of soft tissue sarcomas, contributing to the reduction of the tumor and prevent recurrence of the disease, thus avoiding performing the amputation surgery.</p></abstract><trans-abstract xml:lang="ru"><p><italic>Цель работы. </italic>Оценка эффективности лечения пациентов с местнораспространенными саркомами мягких тканей конечностей с помощью изолированной химиотерапевтической перфузии в условиях гипертермии.<italic>Материалы и методы. </italic>Выполнен анализ результатов лечения 57 больных саркомами мягких тканей конечностей, которые были разделены на 2 группы: основная – 21 больной, получивший лечение с применением изолированной регионарной перфузии (ИРП) мелфалана (в количестве 10 мг/л объема для нижней конечности и 13 мг/л объема для верхней конечности); группа сравнения – 36 пациентов, в лечении которых были использованы стандартные методы с выполнением калечащей операции. При ИРП использовали оригинальную методику мониторинга уровня поступления химиопрепарата в системный кровоток.<italic>Результаты. </italic>Метод изолированной регионарной перфузии у больных с местнораспространенной формой сарком мягких тканей конечностей позволяет достичь общей 2-летней выживаемости 90,5%, в группе сравнения значение показателя составило 75%. Локальная токсичность, связанная с ИРП, в большинстве случаев не превышала I и II степени по шкале Wieberdink. Системная токсичность I–II степени по шкале CTC отмечена в единичных случаях.<italic>Заключение. </italic>Метод ИРП конечности является перспективным методом лечения сарком мягких тканей, способствующим редукции опухоли и предупреждению рецидивов заболевания, что позволяет избежать выполнения ампутационного хирургического вмешательства.</p></trans-abstract><kwd-group xml:lang="en"><kwd>soft tissue sarcoma</kwd><kwd>isolated regional perfusion</kwd><kwd>chemotherapy</kwd><kwd>survival</kwd><kwd>toxicity</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>саркома мягких тканей</kwd><kwd>изолированная регионарная перфузия</kwd><kwd>химиотерапия</kwd><kwd>выживаемость</kwd><kwd>токсичность</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">1.
