Patients were divided into 2 groups: main group – 24 patients received treatment with an isolated regional perfusion (prI), melphalan (10 mg/ l volume of the lower limbs); comparison group – 17 patients are treated with conventional methods with the implementation of mutilation were used. When the prI used the original methodology for monitoring chemotherapy income levels in the systemic circulation.<italic>Results. </italic>It was found that the application of the procedure of isolated regional perfusion for locally disseminated melanoma of the skin contributes to significant increase in survival of patients: the overall two-year survival rate was up to 91,7% compared to 53,8% with standard treatment. local toxicity associated with carrying out the isolated limb perfusion was regional, in most cases did not exceed grade I and II on the extent Wieberdink scale. no cases of systemic toxicity of grade I–II CTC scale were observed, confirming the satisfactory safety of this method in the treatment of cancer.<italic>Conclusion.</italic> method of isolated regional limb perfusion is clinically effective and safe in the treatment of locally disseminated melanoma of the skinlimbs, its use can reduce the incidence of recurrence of the disease and avoid performing the amputation surgery.</p></abstract><trans-abstract xml:lang="ru"><p><italic>Цель исследования. </italic>Оценка эффективности лечения пациентов с меланомой кожи конечностей с помощью изолированной химиотерапевтической перфузии в условиях гипертермии.<italic>Материалы и методы. </italic>Выполнен анализ результатов лечения 41 пациента с диссеминированной меланомой кожи нижней конечности, которые были разделены на 2 группы: основная группа – 24 пациента, получившие лечение с применением изолированной регионарной перфузии (ИРП) мелфалана (в дозе 10 мг/л объема для нижней конечности); группа сравнения – 17 пациентов, в лечении которых были использованы стандартные методы с выполнением калечащей операции. При ИРП использовали оригинальную методику мониторинга уровня поступления химиопрепарата в системный кровоток.<italic>Результаты. </italic>Установлено, что применение процедуры изолированной регионарной перфузии при местно-диссеминированной меланоме кожи конечности способствует значимому повышению выживаемости больных: общая двухлетняя выживаемость достигает 91,7% по сравнению с 53,8% при использовании стандартных методов лечения. Локальная токсичность, связанная с проведением изолированной регионарной перфузии конечности, в большинстве случаев не превышала I и II степени по шкале Wieberdink. Системная токсичность не превышала I–II степени по шкале CTC, что подтверждает удовлетворительную безопасность применения данного метода в лечении онкологических заболеваний.<italic>Заключение. </italic>Метод изолированной регионарной перфузии конечности является клинически эффективным и безопасным при лечении местно-диссеминированной меланомы кожи конечностей, его использование позволяет снизить частоту развития рецидивов заболевания и избежать выполнения ампутационного хирургического вмешательства.</p></trans-abstract><kwd-group xml:lang="en"><kwd>melanoma</kwd><kwd>isolated regional perfusion</kwd><kwd>chemotherapy</kwd><kwd>survival</kwd><kwd>toxicity</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>меланома</kwd><kwd>изолированная регионарная перфузия</kwd><kwd>химиотерапия</kwd><kwd>выживаемость</kwd><kwd>токсичность</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">1.
