It causes significant harm with poor prognosis, and it is very difficult to cure. during the past two decades, the emergence of neoadjuvant chemotherapy of osteosarcoma has greatly improved the survival rate of osteosarcoma patients (5 year survival 60–70%). nearly half of these patients are not sensitive to chemotherapy drugs and die due to lung metastasis within 2 to 3 years.The majority of patients (80%) have small tumor foci transferred into their systemic blood before treatment; the importance of improving long-term survival in patients by controlling lung metastases through effective drug and gene regulation has been increasingly recognized. the invasive ability of tumor cells is one of the important factors in their capacity to metastasize to distant locations in the body. Production of woven bone by tumor cells defines the neoplasm, however, variable amounts of cartilage and fibrous tissue matrix are often encountered as accompanying components. Osteoblastic, chondroblastic and fibroblastic types comprise the 3 major cell types of OS, but the percentage of cell type and matrix will vary in any individual case. The role of the pathologist is becoming more important because of the modern genetic and molecular and IHC markers in diagnostic procedures, the pathologists must rise to the occasion.</p></abstract><trans-abstract xml:lang="ru"><p>Остеосаркома (ОС) – злокачественная опухоль кости, возникающая из костеобразующей мезенхимы и характеризующаяся продукцией атипичного остеоида и костных структур злокачественными пролиферирующими опухолевыми клетками. ОС возникает из мультипотентных клеток мезенхимальной природы, способных дифференцироваться в направлении хряща или кости. и хотя ОС обнаруживает нередко признаки хондросаркомы и фибросаркомы, все эти опухоли, и в первую очередь ОС, необходимо строго дифференцировать между собой. Этиология ОС в большинстве случаев неизвестна. наиболее частой локализацией ОС являются длинные трубчатые кости. основная масса (около 80%) ОС развивается в метафизах длинных трубчатых костей зоны коленного сустава (дистальный метафиз бедренной, проксимальные метафизы болшеберцовой и малоберцовой костей) и проксимальном метафизе плечевой кости. Эти отделы скелета являются в физиологическом отношении наиболее ответственными за рост скелета; именно здесь протекают самые активные процессы ростового остеогенеза; и, по-видимому, именно с ними связана большая вероятность опухолевой трансформации. Для диагностики ОС чрезвычайно важно сопоставление рентгенологической картины очага поражения с морфологическими изменениями, а точнее, обязательный анализ рентгенологической картины перед анализом гистологических или цитологических препаратов. на основании этих совокупных данных выделяют несколько клинико-патологических вариантов ОС. в настоящее время активно ведется изучение остеосарком на генетическом уровне с целью разработки более эффективных методов лечения данной опухоли, поскольку 5-летняя выживаемость при метастатическом распространении остается крайне низкой (20–25%).</p></trans-abstract><kwd-group xml:lang="en"><kwd>osteosarcoma</kwd><kwd>osteoid</kwd><kwd>primary bone sarcoma</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>остеосаркома</kwd><kwd>остеоид</kwd><kwd>первичная саркома кости</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">1.
