MR imaging was performed using a SIeMenS MAgneToМ AVAnTo 1,5T and SKYRA 3T (germany). WBMRI was performed with T1-se and T2 – STIR sequences in coronary projection. We analyzed the sensitivity, specificity, PPV, NPV of WBMRI in detecting metastasis.<italic>Results. </italic>13 patients were determined disseminated forms of soft tissue tumors with presence of the primary tumor, 2 patients had metastases of the primary lesion that was not detected. Skeletal metastases were found in 5 children, of whom 4 in the bone marrow. 7 patients had enlarged lymph nodes, 1 patient in this case had a false positive result on WBMRI. changes in the lung tissue were detected in 3 patients and were confirmed with CT of the chest. In 1 patient with small foci of up to 3 mm, identified in the CT of the chest is given a false negative conclusion on WBMRI. other organs (spleen, peritoneum, pleura, brain), as the metastatic character of malignant forms of soft tissue tumors, and in particular lesion at the neurofibromatosis type 1 is identified at the WBMRI in 3 patients.<italic>Conclusion. </italic>WBMRI showed high information content to identify the primary pathological process and metastatic. Sensitivity of WBMRI and SSC for the detection of bone metastases – 98% and 47.1%, specificity – 100% and 100%, respectively.WBMRI is more informative for the detection metastases of the target organs than abdominal ultrasound: sensitivity – 100% and 66.7%, specificity – 100% and 100%, respectively. WBMRI gives way ultrasound study lymph nodes: sensitivity – 100% and 100%, specificity – 87.5% and 100%, respectively. WBMRI gives way CT of the chest to identify MTS in lung: sensitivity – 75% and 100%, specificity – 100% and 100%, respectively.</p></abstract><trans-abstract xml:lang="ru"><p><italic>Целью работы </italic>явилось улучшение диагностики лимфогенного и гематогенного метастазирования опухолей мягких тканей у детей с диссеминированными поражениями.<italic>Материалы и методы. </italic>Были обследованы 15 детей с диссеминированными формами опухолей. девочек было 8, мальчиков – 7, возраст детей от 1,5 до 16 лет. выявлены злокачественные опухоли мягких тканей у 13 детей (86,7%), у 2% (13,3%) – доброкачественные. исследование проводилось на аппарате Magnetom Avanto 1,5T и Magnetom Skyra 3T, Siemens. стандартный протокол исследования включал в себя две последовательности в коронарной проекции: т1-se и т2 STIR для всего тела. оценивалась информативность МРТ-ВТ в выявлении лимфогенного и гематогенного метастазирования.<italic>Результаты. </italic>у 13 больных определялись диссеминированные формы ОМТ с наличием первичной опухоли, у 2 пациентов имели место метастазы из не выявленного первичного очага. Метастазы в кости скелета были выявлены у 5 детей, из них у 4 – в костный мозг. у 7 пациентов были выявлены увеличенные л/узлы, 1 пациент при этом имел ложноположительный результат по данным МРТ-ВТ. изменения в легочной ткани определялись у 3 пациентов, нашедших подтверждение при РКТ органов грудной клетки. у 1 пациента с наличием мелких очагов до 3 мм, выявленных при РКТ органов грудной клетки, дано ложноотрицательное заключение. поражение других органов (селезенки, брюшины, плевры, головного мозга) как метастатического характера при злокачественных формах опухолей мягких тканей, так и при специфическом поражении при нейрофиброматозе 1-го типа выявлено при МРТ всего тела у 3 больных.<italic>Заключение. </italic>МРТ-ВТ обладает высокими показателями информативности в выявлении как первичного патологического процесса, так и метастатического. информативность МРТ-ВТ и РИД скелета в определении МТС в кости: чувствительность – 98% и 47,1%, специфичность – 100 и 100% соответственно. МРТ-ВТ более информативна при определении поражений органов-мишеней при СМТ, чем узи брюшной полости: чувствительность – 100 и 66,7%, специфичность – 100 и 100% соответственно. МРТ-ВТ уступает уз-исследованию л/узлов: чувствительность – 100 и 100%, специфичность – 87,5 и 100% соответственно.
