Gastrointestinal stromal tumors are the most frequent mesenchymal tumors that are characterized by mutations in tyrosine kinase receptors KIT and PDGFRA. Mutations in KIT and PDGFRA genes determine the sensitivity to therapy with tyrosine kinase inhibitors and are associated with prognosis of GIST patients. Analysis of the mutational status of 52 GIST patients with localized disease treated in N.N. Blokhin RCRC between 2001 and 2007 was conducted. KITmutations were found in 40 patients (76,9%), of which 34 cases (65,5%) exhibited 11 exon, 5 (9,6%) 9 exon and 1 (1,9%) case 13 exon. PDGFRA mutations were detected in 18 exon in 7 (13,5%) patients. WT genotype was found in 5 (9,6%) patients. Deletions in KIT exon 11 were detected in 17 c10 better clinical outcome (5-year survival rate of 100,0% and 75,0%) when compared with deletions in KIT exone 11 and duplications in exon 9 (5-year survival rate of 48,4% and 25,0%). Survival analysis with deletions KIT11 exon showed no significant difference of survival rate in patients with gastric and intestinal stromal tumors.</p></abstract><trans-abstract xml:lang="ru"><p>Анализ мутационного статуса у больных гастроинтестинальными стромальными опухолями (ГИСО) имеет большое клиническое и прогностическое значение. Гастроинтестинальные стромальные опухоли являются наиболее распространенными мезенхимальными опухолями ЖКТ, которые отличаются наличием мутаций в генах KIT и PDGFRA, кодирующих тирозинкиназные рецепторы. Тип мутации определяет чувствительность к ингибиторам тирозинкиназ и связан с прогнозом. Нами проведен анализ и оценка прогностической значимости мутаций генов KIT и PDGFRA у 52 больных с локализованными ГИСО, которым проводилось лечение в РОНЦ им. Н.Н. Блохина в 2001-2007 гг. KIT мутации были обнаружены у 40 (76,9%) пациентов, из них 34 (65,5%) имели мутации в 11-м экзоне, 5 (9,6%) в 9-м экзоне и 1 (1,9%) в 13-м экзоне. PDGFRA мутации выявлены только в 18-м экзоне у 7 (13,5%) больных. У 5 больных (9,6%) мутации в генах KIT и PDGFRA не обнаружены. Делеции KIT в 11-м экзоне KIT обнаружены у 17 пациентов, реже встречались точечные замены у 14 больных и дупликации у 3 больных. Дупликации и точечные замены в 11-м экзоне KIT ассоциированы с благоприятным прогнозом (5-летняя выживаемость без лечения Гливеком составила 100 и 75% соответственно), тогда как делеции в 11-м экзоне KIT и дупликации в 9-м экзоне KIT с плохим.
