Aim. To assess possible influence of surgical margins on local recurrence rate and survival. Methods. Retrospective analysis of the individual data of 352 patients with primary cutaneous melanoma of the trunk and limbs was performed. 108 patients underwent so-called «wide excision» of the tumor with the average margins from the visible tumor edges of 3,5 cm. In 244 patients more «narrow» excision was performed with average margins 2,0 cm. Results and conclusion. Margins had not any influence on treatment results. The frequency of local recurrence, regional and distant metastases, as well as 5-year survival rates were similar in both groups.

Цель работы. Оценить влияние отступа от видимого края опухоли на результаты лечения. Материалы и методы. В работе при помощи ретроспективного анализа данных о 352 больных с первичной меланомой кожи туловища и конечностей изучены отдаленные результаты хирургического лечения. 108 больным было проведено т. н. широкое иссечение опухоли со средним отступом от ее краев на 3,5 см. У 244 больных средняя величина отступа была более экономной и составила 2 см. Результаты и выводы. Сокращение объема операции на первичном очаге меланомы кожи не отразилось на показателях лечения. Частота местного рецидива, регионарных и отдаленных метастазов, а также цифры 5-летней выживаемости были идентичными в обеих группах.
