Success in early diagnostic and system treatment is achieved. All the same mastectomy with reconstruction is the best option for some patients. Seroma is common complication after reconstructive plastic surgery.<bold>Aim. </bold>To analyze efficiency of using fibrin sealant criofit at donor zone after autologous breast reconstruction in decreasing seroma incidences.<bold>Materials and methods. </bold>This is randomised controlled clinical trial of fibrin sealant criofit effect on assessing of seroma incidences, timing of drain removal at donor zone after autologous breast reconstruction in comparison with control group. Surgical technic, drains removal criteria and patients characteristics were identical in both groups.<bold>Results. </bold>Average drain fluid volume on the first day (150.9 ± 40.7 ml <italic>vs </italic>190.6 ± 60.7 ml; <italic>p </italic>&lt;0.001) and second day (152.6 ± 53.3 ml <italic>vs </italic>184.9 ± 90.3 ml; <italic>p </italic>= 0.04) after surgery was significantly lower in experimental group. Average number of days of drainage in criofit group was significantly lower (6.3 ± 1.1 days <italic>vs </italic>7.4 ± 2.1 days; <italic>p </italic>= 0.01). Seroma incidences in experimental group was lower in comparison with control group (10 % <italic>vs </italic>23 %). It was statistically significant.<bold>Conclusion. </bold>We aimed to asses the efficiency of fibrin sealant criofit at donor zone after autologous breast reconstruction. Criofit decrese the volume of serous draining in first days after surgery, number of days of drainage and seroma incidences significantly. But more powerful clinical trials are needed. Study limitations are small sample, changes in surgery team, small observation period.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение. </bold>Рак молочной железы – распространенное онкологическое заболевание. Несмотря на успехи в ранней диагностике и системном лечении данной патологии, для значительной части пациенток операцией выбора служит мастэктомия с последующей реконструкцией. Одним из частых осложнений реконструктивно пластической хирургии молочной железы является серома донорской зоны.<bold>Цель исследования </bold>– проспективный анализ эффективности (снижение частоты развития сером и объема отделяемого) использования в области донорской зоны фибринового герметика криофит при аутологичной реконструкции молочной железы.<bold>Материалы и методы. </bold>В данном рандомизированном контролируемом клиническом исследовании оценивали влияние фибринового герметика криофит на частоту развития сером и сроки удаления дренажей в донорской зоне при аутологичной реконструкции молочной железы. Оперативная техника, критерии удаления дренажей, характеристика пациентов были идентичными в группах сравнения.<bold>Результаты. </bold>Средний объем дренажной жидкости в 1-й послеоперационный день в группе криофита составил 150,9 ± 40,7 мл, в группе контроля – 190,6 ± 60,7 мл; <italic>p </italic>&lt;0,001), во 2-й послеоперационный день – 152,6 ± 53,3 и 184,9 ± 90,3 мл соответственно (<italic>p </italic>= 0,04). Таким образом, этот показатель был значительно ниже в экспериментальной группе. Средний срок удаления дренажей в группе криофита оказался значительно меньше, чем в контрольной группе: 6,3 ± 1,1 и 7,4 ± 2,1 соответственно (<italic>p </italic>= 0,01). Частота возникновения серомы в экспериментальной группе (10 % случаев) оказалась ниже, чем в контрольной группе (23 % случаев); данная разница статистически значима.<bold>Заключение. </bold>Использование фибринового герметика криофит в донорской зоне при аутологичной реконструкции молочной железы существенно снижает серозное отделяемое в первые послеоперационные дни, уменьшает время использования дренажа, а также частоту развития сером. Ограничениями исследования были небольшие размер выборки и продолжительность наблюдения, а также изменения в составе операционной бригады.
