Our study is based on our experience with biological reconstruction methods used during such operations. We have developed both the methodology and necessary tools for its implementation, as well as conducted a comprehensive evaluation of its practical application results.</p><p><bold>Aim</bold>. To assess the effectiveness of using biological bone reconstruction techniques in oncologic revision knee arthroplasty.</p><p><bold>Materials and methods</bold>. A retrospective study included 57 patients who underwent revision total oncology knee arthroplasty with the use of biological reconstruction for intramedullary bone defects between 2017 and 2023, with a follow-up period of at least 12 months. Of these, 45 (78.9 %) were operated due to instability of the prosthetic components, while 12 (21.1 %) were treated in the second stage after spacer implantation due to infection.</p><p>Distribution of patients by location of bone defect: 26 (45.5 %) had femoral bone defects, 4 (7 %) had tibial bone defects, and 27 (47.4 %) had combined lesions involving both bones. Among the participants, 59.6 % were female and 40.4 % were male. The median age was 48 years (Q<sub>1</sub>–Q<sub>3</sub>) 39.00–57.00; min 23 – max 72), and the median follow-up time was 48 months (Q<sub>1</sub>–Q<sub>3</sub> 28.00–60.00; min 12 – max 150). Median blood loss values were 900 ml (Q<sub>1</sub>–Q<sub>3</sub> 700.0–1100.0; min 400 – max 2200), operation duration was 220 minutes (Q<sub>1</sub>–Q<sub>3</sub> 190.0–240.0; min 140 – max 300), and patient weight was 85 kg (Q<sub>1</sub>–Q<sub>3</sub> 77.0–95.0; min 56 – max 180).</p><p>We assessed complication risks, functional outcomes, and survival rates of the implants. All patients were divided into two groups: Group 1 (n = 37 or 64.9 %), operated before the introduction of new instruments, and Group 2 (n = 20 or 35.1%), operated after their introduction. Parameters such as operation duration and blood loss volume were compared between the groups.</p><p><bold>Results.</bold> Over the entire observation period from 2017 to 2024, three cases (5.3%) of complications were identified. Bone density assessment in the bone graft zone was performed via CT scan six months postoperatively, yielding a mean value of 690 HU (Q<sub>1</sub>–Q<sub>3</sub> 570.0 – 790.0; min 340 – max 980).Analysis of lower limb function dynamics according to the MSTS scale pre- and post-operation revealed statistically significant improvements (p &lt;0.001) (using Wilcoxon’s test).Five-year survival rate of knee prostheses after revision arthroplasty was 96 %.To analyze the effectiveness of introducing the newly developed instrumentation, we studied blood loss and operative times with and without its use. Using the instrumentation significantly reduced blood loss (p &lt;0.001) and operative time (p &lt;0.001) as determined by Mann-Whitney U-tests.</p><p><bold>Conclusion.</bold> The five-year survival rates for constructs in patients undergoing surgery with biological bone reconstruction are comparable to those observed after primary implantations of oncologic prostheses.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> С увеличением частоты ревизионных операций по замене онкологических эндопротезов коленного сустава одной из наиболее значимых трудностей стала необходимость устранения значительных внутрикостных дефектов, затрагивающих диафизы и метаэпифизы костей. Наше исследование основывается на собственном опыте применения методов биологической реконструкции костной ткани при проведении подобных операций. Мы разработали методику и необходимый для ее осуществления инструментарий, а также провели всестороннюю оценку результатов ее практического применения.</p><p><bold>Цель исследования</bold> – оценить эффективность применения биологической реконструкции кости при выполнении онкологического ревизионного эндопротезирования коленного сустава.</p><p><bold>Материалы и методы</bold>. В ретроспективное исследование были включены 57 пациентов, перенесших ревизионное тотальное онкологическое эндопротезирование коленного сустава с использованием метода биологической реконструкции внутрикостных дефектов в период с 2017 по 2023 г. со сроком наблюдения не менее 12 мес. Из них 45 (78,9 %) пациентов оперировали из-за нестабильности компонентов эндопротеза, а 12 (21,1 %) – на 2-м этапе после установки спейсера по поводу инфекции.</p><p>Распределение пациентов по локализации костного дефекта: 26 (45,5 %) имели дефекты бедренной кости, 4 (7 %) – дефекты большеберцовой кости и 27 (47,4 %) – комбинированные поражения обеих костей. Среди участников исследования 59,6 % составляли женщины, 40,4 % – мужчины. Медиана возраста пациентов – 48 лет (Q<sub>1</sub>–Q<sub>3</sub> 39,00–57,00; min 23 – max 72), медиана срока наблюдения – 48 мес (Q<sub>1</sub>–Q<sub>3</sub> 28,00–60,00; min 12 – max 150). Медианные значения кровопотери составили 900 мл (Q1–Q3 700,0–110,00; min 400 – max 2200), длительность операции – 220 мин (Q<sub>1</sub>–Q<sub>3</sub> 190,0–240,00; min 140 – max 300), масса тела пациента – 85 кг (Q<sub>1</sub>–Q<sub>3</sub> 77,0–95,00; min 56 – max 180).Оценивали риски осложнений, функциональные результаты и сроки выживаемости эндопротезов.</p><p>Все пациенты были разделены на 2 группы: 1-я группа (n = 37, или 64,9 %) – прооперированные до внедрения нового инструментария, и 2-я группа (n = 20, или 35,1 %) – прооперированные с его использованием.</p><p>При сравнении групп анализировались такие параметры, как продолжительность операции и объем кровопотери.</p><p>Также был использован разработанный и запатентованный набор инструментов для биологической реконструкции длинных костей при ревизионном онкологическом эндопротезировании коленного сустава (патент № 2804798 C1).</p><p><bold>Результаты.</bold> За весь период наблюдения, с 2017 по 2024 г., выявлено 3 (5,3 %) случая осложнений. Два из них связаны с глубокой инфекцией области хирургического вмешательства, развились на сроке 71 и 26 мес соответственно, и у 1 пациента произошел травматический многооскольчатый перепротезный перелом бедренной кости на фоне падения с высоты через 22 мес после операции. Оценку плотности кости в зоне костной пластики осуществляли при помощи компьютерной томографии на сроке 6 мес, по шкале Хаунсфилда Ме 690 (Q<sub>1</sub>–Q<sub>3</sub> 570,0–790,00; min 340 – max 980).Нами был проведен анализ динамики функции нижней конечности по шкале MSTS до и через 12 мес после операции, выявлено их статистически значимое улучшение (p &lt;0,001) (использован критерий Уилкоксона).Пятилетняя выживаемость эндопротезов коленного сустава после проведенного ревизионного эндопротезирования составила 96 %.С целью анализа эффективности внедрения в практику разработанного инструментария были изучены объем кровопотери и время операции в зависимости от использования инструментария. Применение инструментария позволило статистически значимо снизить кровопотерю (p &lt;0,001) (использован U-критерий Манна–Уитни) и время операции (p &lt;0,001) (использован U-критерий Манна–Уитни).</p><p><bold>Заключение.</bold> Пятилетние показатели выживаемости конструкций у пациентов, перенесших операцию с использованием биологической реконструкции костей, демонстрируют результаты, сравнимые с теми, что наблюдаются после первичной имплантации онкологического эндопротеза коленного сустава.
