Gene expression profiling potentially allows for more accurate risk stratification of patients.</p><p><bold>Aim.</bold> To study prognostic test systems for assessing outcomes in patients with skin melanoma based on the analysis of primary tumors.</p><p><bold>Materials and methods</bold>. A systematic literature review (scoping review) was conducted in accordance with PRISMA-ScR principles. The search was performed in PubMed (2008–2024). Two independent reviewers conducted the study selection and data analysis to assess concordance. The data were presented descriptively.</p><p><bold>Results</bold>. Out of 149 identified publications, 31 studies were included in the review. The effectiveness of four test systems was evaluated, with the most frequently used being DecisionDx-Melanoma (19/31, 61.3%). This test stratifies patients by molecular classes: patients at high risk were found to have a 5.33 (±1.25) times higher likelihood of disease progression and poorer survival rates compared to lower-risk patients. No studies included data on the Russian population.</p><p><bold>Conclusions</bold>. Gene expression profiling demonstrates high accuracy in predicting outcomes for patients with skin melanoma.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение</bold>. Меланома кожи, несмотря на схожие клинические и гистологические характеристики, может иметь разный прогноз. Профилирование экспрессии генов потенциально позволяет более точно стратифицировать больных по группам риска.</p><p><bold>Цель исследования</bold> – изучение тест-систем для прогнозирования исходов у пациентов с меланомой кожи на основе анализа первичной опухоли.</p><p><bold>Материалы и методы</bold>. Проведен систематический обзор литературы (scoping review) в соответствии с принципами PRISMA-ScR. Поиск выполнен в PubMed (2008–2024). В обзор включено 31 исследование из 149 выявленных публикаций. Отбор исследований и анализ данных осуществляли два независимых рецензента для оценки конкордантности. Данные представлены в описательной форме.</p><p><bold>Результаты.</bold> Оценена эффективность четырех тест-систем, включая наиболее часто используемую DecisionDx-Melanoma (19/31, 61,3 %). Данный тест стратифицирует пациентов по молекулярным классам: у пациентов с высоким риском прогрессирование отмечается в 5,33 (±1,25) раза чаще и сопровождается худшими показателями выживаемости.
