Aggressive benign spinal tumors in children frequently require extensive surgical resection necessitating complex reconstructive interventions. Currently, vertebrologists have at their disposal a large number of prostheses for replacement of the anterior supporting column. Both static implants and extendable spacer are used. However, these implants have limitations which can affect long-term results especially in children when growth should be taken into account and long-term stability of the prosthesis should be assured. Design of personalized titanium prostheses adapted to anatomical features of the vertebrae allows to solve these problems.</p><p><bold>Aim</bold>. To evaluate the effectiveness of using personalized titanium vertebral corpectomy prostheses for reconstruction of postresection defects in children with aggressive benign spinal tumors (stage III per the Enneking classification).</p><p><bold>Materials and methods.</bold> The study included 5 patients of pediatric age who underwent two-stage surgical intervention including dorsal stabilization, tumor resection and installation of a personalized titanium vertebral corpectomy prosthesis manufactured using direct metal laser sintering (DMLS) 3D printing.</p><p><bold>Results</bold>. No significant complications were observed in the postoperative period. In the first 3 months, neurological status was completely restored in all patients. Computed tomography showed stable implant integration; minimal signs of prosthesis subsidence (1 mm without signs of resorption around screw implants) were observed only in 1 case.</p><p><bold>Conclusion</bold>. The use of personalized titanium vertebral corpectomy prostheses improves the results of surgical treatment of spinal tumors in children. The proposed innovative technique of postresection defect replacement with personalized implants manufactured by 3D printing increases spinal fixation stability after extensive tumor resection and decreases the risk of complications associated with implant instability. The use of additive methods of prosthesis manufacturing allows to solve complex reconstruction problems in spinal surgical interventions in children.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Агрессивные доброкачественные опухоли позвоночника у детей часто требуют обширной хирургической резекции, что приводит к необходимости проведения сложных реконструктивных вмешательств. В настоящее время в арсенале вертебрологов имеется большое количество протезов, позволяющих заместить переднюю опорную колонну. При этом используют как статичные моделируемые имплантаты, так и раздвижные спейсеры. Однако эти имплантаты имеют ограничения, которые могут повлиять на долгосрочные результаты, особенно у пациентов детского возраста, когда требуется учитывать рост и обеспечивать долговременную стабильность протеза. Разработка индивидуальных титановых протезов, адаптированных к особенностям анатомии позвонков, позволяет решить данные проблемы.</p><p><bold>Цель исследования</bold> – оценка эффективности применения индивидуальных титановых протезов тел позвонков для реконструкции пострезекционных дефектов при агрессивных доброкачественных опухолях позвоночника (III стадия по классификации Enneking) у пациентов детского возраста.</p><p><bold>Материалы и методы</bold>. В исследование вошли 5 пациентов детского возраста, которым проведено двухэтапное хирургическое вмешательство, включающее дорзальную стабилизацию, резекцию опухоли и установку индивидуального титанового протеза тела позвонка, изготовленного с помощью 3D-печати методом прямого лазерного спекания металла (DMLS).</p><p><bold>Результаты</bold>. В послеоперационном периоде значительных осложнений не наблюдалось. В течение первых 3 мес неврологический статус был полностью восстановлен у всех пациентов. Результаты компьютерной томографии показали стабильную интеграцию имплантата, при этом минимальные признаки его проседания (1 мм без признаков резорбции вокруг винтовых имплантатов) наблюдались только в 1 случае.</p><p><bold>Заключение</bold>. Применение индивидуальных титановых протезов тел позвонков обеспечивает улучшение результатов хирургического лечения опухолей позвоночника у детей. Предложенный инновационный метод замещения пострезекционных дефектов с применением индивидуальных имплантатов, изготовленных методом 3D-печати повышает стабильность фиксации позвоночника при обширной резекции опухолей и снижает риск развития осложнений, связанных с нестабильностью имплантатов.
